We've finished our 2020 Clinic!
Get Your Free Flu Shot!
Everyone 6 months and older should be vaccinated.
No ID nor health insurance required, this program is completely free and open to the public.
HOWEVER, YOU MUST SIGN UP FOR AN APPOINTMENT IN ADVANCE TO GET YOUR FLU SHOT. SEE AVAILABLE DATES AND TIMES BELOW.

All days have appointment slots from 10:00 AM to 4:00 PM unless otherwise noted. To abide by government health regulations, appointment slots are limited and will be made available on a first come, first serve basis.
If a certain day is not yet underlined (no link), then we are not yet accepting appointments for that day. Please check the site again in the near future for those forms to become available.
If you do not see a particular time slot available for a certain day, it has already been taken. Feel free to check back in the future, more slots may become available as people cancel/reschedule their appointments.
If all appointments are taken for a certain day (only choice is "No options left"), you may sign up as a walk-in, but we will only accommodate you if there is space available and it is safe to do so, and only between 10:00 AM and 1:00 PM. If you do decide to walk in, we highly recommend printing out and filling in the required waivers below, to expedite your processing.

Know Before You Go
Preparation and Arrival
Everyone in the car above the age of three should bring a face mask, and wear it while they are on site at the Health Fair.
Those receiving flu shots should wear clothing with easy access to the upper arm.
As our clinic is outdoors, inclement weather or other unforeseen circumstances may force us to close the clinic early. Please check this page on our website before you come, in case of sudden cancellations.
At your dedicated appointment time, come to the Samaritan House at 4031 Pacific Blvd, San Mateo, CA 94403.
IF YOU ARE FEELING UNWELL, DO NOT COME TO YOUR APPOINTMENT. CANCEL IT AND RESCHEDULE IT FOR WHEN YOU ARE ABLE TO CLEAR THE COVID-19 SYMPTOM SCREENING QUESTIONS LISTED BELOW.
Waivers
Anyone receiving a flu shot will have to fill out the following waivers, which will be available during on site registration. Any child under the age of eighteen must have their parent or legal guardian present to sign for them.
You can save time and effort on site by printing and filling out the waivers beforehand.
All fields requesting for signature of person receiving a flu shot must be signed by a child's parent or guardian if under the age of 18.
One per person receiving flu shot, only first page needs to be printed
One per group/car (fill one line per person receiving flu shot)
If you have children under 3 or pregnant women in your group, put them together on a separate form.
One per group/car receiving flu shot. Head of household should fill the top section, and other members listed as members of the household.
